LITTLE HOUSE NEEDLEWORKS - ORNAMENT-OF-THE-MONTH AUTO SHIP SIGN-UP

If you are interested in subscribing to the Little House Needleworks Ornament-of-the-Month Automatic Program, please fill out the following form with the requested information. The following pricing information is based on the supplies for the first ornament. The pricing for the subsequent ornaments will be similar.
Patterns - $6.00 each
Fabric (30 count Weeks Dye Works Cappuccino Linen) - $3.00 each
Floss (DMC - 7 skeins) - $2.80 ($0.40 per skein)
Beads (Mill Hill - 1 package) - $1.24 each
Shipping cost will be $2.00 flat rate for each automatic order shipment. You may add as many additional items as you like to each order to take advantage of the half-price shipping.
